Care Partner - Clinical - what's involved in the role?


The role of Care Partner (Clinical) works within the Mission and Mission in Action of HammondCare and plays a pivotal role in supporting older adults to live independently and well in their own homes. The role is responsible for responsible for client centred care planning and clinical care management, and supporting the Client Care Operations Manager in achieving strategic objectives and growing the clients we care for at HammondCare.


This role requires a deep understanding of aged care services, a client-centred approach, and strong communication and coordination skills. You will work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans that align with each client's unique needs, preferences, and goals.


Key Focus Areas:

  • Assess and understand individual client needs, including clinical needs and preferences to develop tailored care plans that align with service expectations

  • Coordinate and oversee client centred care planning to ensure services are appropriate, responsive and person-focused

  • Manage and monitor the delivery of in-home support services ensuring compliance to Aged Care Quality Standards

  • Build and maintain strong client relationships by providing ongoing support, communication and guidance throughout their care journey

  • Collaborate with multidisciplinary teams to document, review and ensure compliance with care plans and all related activities


What do you need to apply?

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.

  • University-level qualification in a relevant health-related discipline, for example, Bachelor of Social Worker, Physiotherapy or Nursing.

  • Experience in aged care, home care, or community care, with a passion for client-focused service delivery.

  • Excellent communication and relationship-building skills, with a commitment to advocacy and person-centred care

  • Knowledge of Aged Care Quality Standards and compliance requirements.

  • Experience working collaboratively with healthcare professionals, carers, and external providers.

  • Strong problem-solving, organisational, and coordination skills.

  • Experienced in operating effectively in a multi-site environment.

  • A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s sites and services.

Essential Requirements

This role will see you working in a remote Aboriginal community. All applicants must have an understanding of and commitment to Aboriginal Community Control, self-determination, and the issues faced by YolÅ‹u people living in remote settings. To apply, please ensure you have a current Registration and Practicing Certificate as Registered Nurse through AHPRA; a current Remote Emergency Care Course certificate and/or Advanced Life Support certificate; a current accredited immunisation certificate (recognised in the NT); a Remote Pharmacotherapeutic Certificate (or equivalent); manual driver’s license; and 3 – 5 years post-graduate experience with advanced clinical skills and competencies applicable to remote settings (including the ability to triage, work independently, and participate in afterhours emergency calls and First Response).

About the Role:
