  • Is an experienced Registered Nurse with strong aged care expertise.

  • Can collaborate with diverse teams and you understand the value of community connections in supporting older people to live well at home.
  • Has experience in community nursing, case management, or care coordination is a plus but not essential.

About the Role  

The Diabetes Educator provides culturally safe, person-centred diabetes education and self-management support to Aboriginal and Torres Strait Islander individuals, families, and communities. Working collaboratively within a multidisciplinary team, the educator delivers clinical care, education, and advocacy to support improved diabetes outcomes and reduce the burden of disease.

 

About You

To excel in this position, you should possess the following qualifications and qualities:

  • Division 1 Registered Nurse with current Registration with Australian Health Practitioner Regulation Agency (AHPRA) in Victoria.

  • ADEA Credentialed Diabetes Educator.

  • Demonstrated experience working with Aboriginal and Torres Strait Islander peoples and a strong understanding of the cultural, social, and historical determinants of health.

  • Ability to deliver diabetes education in a culturally responsive, trauma-informed, and strengths-based manner.

  • Excellent communication and interpersonal skills, including the ability to build relationships with clients, families, and community leaders.

  • Commitment to working within a multidisciplinary team and contributing to culturally safe and inclusive practice.

About the Role:

Capecare is looking for a passionate and experienced Physiotherapist to promote the health and well-being of clients transitioning from hospitals into short-term care with Capecare.

This role will be responsible for assessing, planning and implementing physiotherapy treatment for the clients based on care needs.

This role will strive to maximise client outcomes and support clients to achieve their goals and maintain optimal independence, with a focus on improving mobility, strength and overall functional capacity.

There may also be additional support required across both our residential facilities in Busselton and Dunsborough.

Your key responsibilities will include:

  • Conduct comprehensive assessments for clients across both short-term and residential.

  • Assessing for physical health including mobility, strength and cardiovascular fitness.

  • Based on assessment outcomes, prescribe necessary aids or structural modifications to enhance mobility and independence.

  • In consultation with the Facility Manager, and other health care professionals, participate in the development and review of client care plans.

  • Develop and implement individualised physiotherapy programs based on assessment findings and client health goals.

  • Monitor and track client progress, adjusting physiotherapy programs as needed to ensure optimal outcomes.

  • Provide guidance to the Allied Health Assistants to ensure best practice allied health programs are delivered to clients.

  • Provide education to clients and their families on the benefits of regular allied health programs for maintaining and improving health outcomes.

  • Deliver staff training on manual handling.

  • Remain up to date with the latest research and best practices in physiotherapy, incorporating evidence-based approaches into daily practice.

About You:

You’re a compassionate and dedicated Physiotherapist who’s passionate about helping older adults regain their strength, confidence, and independence.
With a strong clinical background and a client-centred approach, you conduct comprehensive assessments, develop tailored treatment plans, and collaborate with multidisciplinary teams to achieve positive outcomes. Ideally, you bring a solid understanding of the Aged Care Standards, excellent communication skills, and a commitment to ongoing learning and evidence-based practice.

To be successful in this role, you’ll need:

  • Bachelor’s degree in Physiotherapy, or related field.

  • Current registration with the Australian Health Practitioner Regulatory Agency (AHPRA).

  • Minimum of 3 years working as a qualified Physiotherapist

  • Experience in the aged care sector (beneficial not essential)
