About the Role
As a Carer Support Planner, you’ll provide personalised, strength-based support to unpaid Carers. Using your communication skills, empathy, and knowledge of community services, you’ll guide Carers through the intake, assessment, and planning process—then help them access services tailored to their needs.
This role is ideal for someone who thrives in a people-focused, fast-paced environment and is passionate about making a genuine difference in people’s lives.
Key responsibilities (not limited to):
Conduct Carer intakes, registrations, assessments, and reviews using the Carer Star™ and other person-centred tools
Provide timely and appropriate referrals to Carer Gateway services and external providers
Create outcome-focused support plans and advocate for Carers when needed
Write clear, goal-based case notes and maintain accurate records
Collaborate across internal teams and partner agencies to ensure continuity of care
Provide backup support to the Triage team during high demand periods
Participate in community engagement activities and support Carers at events
About You
You’re a compassionate and organised professional who can confidently support clients via phone, email, or in person. You understand the challenges Carers face and are motivated by helping others navigate complex service systems.
You’ll bring:
Experience in community services, telephone counselling, or case coordination
Strong communication, time management, and problem-solving skills
An understanding of person-centred and strength-based frameworks
The ability to work flexibly, including extended hours on a rostered basis
Proficiency in CRM and case management systems
A collaborative and culturally respectful approach to working with Carers
Requirements
Tertiary qualification in Social or Community Services, or relevant equivalent experience
Current DHS Working with Children Check
Current National Police Check
Mandatory child protection training (or willingness to complete)
