At UnitingSA, we’re looking for passionate and experienced Registered Nurses to join our Community Aged Care team. This is more than a clinical role, it's a chance to work closely with clients, build trust over time, and deliver truly person-centred care that supports independence, dignity and wellbeing at home.

What You'll Be Doing

As a Community Registered Nurse, you’ll travel across metro Adelaide to deliver exceptional person-centred care, that empowers clients to live their best lives and continue thriving in the comfort of their own homes.

With the support of your experienced and friendly Clinical Nurse Specialist, you will;

  • Provide hands-on clinical care including dementia care, medication management, wound care and chronic disease management
  • Undertake comprehensive and holistic assessments and contribute to the client’s individual care plan
  • Monitoring and documenting clients' health status and progress, including reporting of signs of clinical deterioration of clients
  • Support early intervention strategies to reduce hospital admissions
  • Build strong, supportive relationships and communicate effectively with clients, families, care providers and health professionals to establish goals, provide support and education
  • Educate and empower clients to manage their health with confidence
  • Promote dignity, autonomy and choice in every interaction
  • Work autonomously while staying connected to a supportive team of nurses and health professionals
  • Ensure compliance with Aged Care Standards and clinical governance requirements.

What You Bring

  • Registration with AHPRA as a Registered Nurse
  • A Bachelor of Nursing and current First Aid Certificate
  • Recent nursing experience in aged care, community or home care settings
  • Strong clinical skills in chronic disease management, wound care and dementia
  • Strong clinical knowledge and understanding of contemporary nursing practices
  • Excellent interpersonal, communication and time management skills.
  • Ability to work independently and as part of a united team
  • A warm, respectful communication style and a deep respect for older people
  • A current SA driver’s licence, your own reliable vehicle and comprehensive car insurance
  • A current NDIS Worker Screening Check (or willingness to obtain)
  • Proficiency with clinical documentation systems
