We're looking for passionate and committed Case Managers to join the Relink & Reconnect program, supporting Aboriginal women and trans and gender diverse people exiting prison. This role plays a vital part in breaking the cycle of incarceration by delivering holistic, culturally safe, and trauma-informed support.
You'll work as part of a multidisciplinary team providing intensive case management, advocacy, and referral pathways while supporting clients to strengthen their social and emotional wellbeing. The role also offers the opportunity to contribute to sector development through secondary consultation, training, and co-working with partner organisations.
- Providing pre and post release case management and transition planning
- Undertaking risk and safety screening, crisis intervention, and referrals
- Working closely with Corrections Victoria, justice services, and community supports
- Building strong collaborative relationships with partner services
- Supporting the empowerment of Aboriginal women, families, and communities
- A degree in Social Work or related field (or significant relevant experience)
- Strong understanding of systemic issues impacting justice-involved women and trans and gender diverse people
- Experience providing culturally appropriate crisis intervention and case management
- Knowledge of MARAM, FVISS, and CISS frameworks
- A commitment to Aboriginal self-determination and community-led practice
