About the Role:
Echuca Regional Health currently have a vacancy for a Credentialled Diabetes Educator. This role provides expert advice and support across the health service in diabetes management in both inpatient and outpatient settings. The role is varied in nature and tasks include, but are not limited to, outpatient clinic, inpatient support, assistance in the upskilling of staff as well as participating and running group education programs as required.
Key responsibilities include:
Conduct assessments of individuals, considering disease type and stage, treatment, social situation, physical function, psychosocial needs and other relevant clinical factors
Participate in and lead quality improvement activities, while fostering a culture of continuous improvement
Actively participate in Nutrition and Dietetics and/ or Diabetes Education team meetings, as well as program or unit meetings, as delegated
Provide client-focused clinical services in collaboration with the multidisciplinary team
Participate in and contribute to the supervision of students and support ongoing nursing education, as applicable
Qualifications/experience required:
Post Graduate Certificate in Diabetes Education
Credentialed with Australian Diabetes Educators Association
Desirable:
3+ years’ experience working in Diabetes Education
Experience with insulin pump therapy (CSII) and continuous glucose monitoring systems (CGMS)
Experience with complex diabetes management cases, RCP, Patient Flow Manager and BOS
