The opportunity
The Silverchain Liaison Nurse will promote services and products, facilitate referrals and will liaise with clients, families, hospitals, other community health care providers and employees, to ensure a smooth transition of client care. Provide a liaison function with the nominated Health Network to build strong a partnership between the hospital system and Silverchain WA. 8.00am - 4.00pm (no weekends and Public Holidays) This role will see you working from any allocated Perth Metro Hospitals when required and Office based in Osborne Park. 60.8 hours minimum per fortnight.
You will be an experienced Registered Nurse with Strong communication skills (written, verbal, listening and telephone) and the proven ability to liaise effectively to build and maintain relationships with all stakeholders and provide appropriate education and information.
