Remuneration value up to $172,272 p.a. comprising salary between $5,699 p.f. - $6,603 p.f. (L4 - L9) employer superannuation contribution (up to 12.75%), annual leave loading (17.5%), professional development allowance, professional development leave 1.6 weeks p.a.
About the Unit
The Care in the Right Setting (CaRs) program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS Team includes Hospital in The Home (HiTH), Geriatric Evaluation Management / Rehabilitation – Hospital in The Home (GEM/R-HiTH), and Residential Aged Care Facility Support Service (RaSS) and Geriatric Emergency Department Intervention (GEDI) care providers. Patients receive quality care that is tailored to their individual health care needs.
The GEM/R-HiTH service provides care to admitted inpatients in their homes or suitable accommodation. This sub-acute service, led by a geriatrician and rehabilitation physician, facilitates multidisciplinary community-based care, enabling a seamless transition home. It supports a range of patients, including those with orthopaedic, stroke, respiratory, and chronic conditions, through short-term rehabilitation aimed at maximising functionality in their own environment. This care is delivered by medical staff, along with nurses, physiotherapists, occupational therapists, social workers, dietitians and speech pathologists.
The RaSS model provides care in partnership with residential aged care facilities and GPs to provide care in nursing homes.
